Dental fillings: what to do about decay or a hole in a tooth

Medically reviewed by Dr. Atoshe Islam7 November 2026

Tooth decay may not hurt at first. Filling a hole while it is small can often avoid bigger treatment.

Signs to get checked

  • Toothache.
  • Sharp pain with hot, cold or sweet food and drink.
  • White, brown or black spots on a tooth.
  • A hole in a tooth, or food getting stuck there.

Decay can be present with no symptoms, so regular check-ups matter.

What happens in a filling

A filling repairs a hole caused by decay. There are different types, such as white fillings. The dentist chooses what suits your tooth.

What happens if you wait

As decay grows, the tooth or gum can become infected and pus can collect. If decay reaches the soft inner part of the tooth, a root canal may be needed. Sometimes the tooth has to come out.

How to prevent decay

  • Brush with fluoride toothpaste at least twice a day.
  • Spit after brushing. Don't rinse with water.
  • Clean between your teeth every day.
  • Cut down on sweet food and drinks.
  • Don't smoke.
  • Come for regular check-ups.
